Apple Systems
The cause on Apple systems could be your USB cable so check the cable for damages. Often, if you unplug the USB cable at both ends, it will solve the issue. Otherwise, the cable may need to be replaced so check the device and system’s USB slots for debris from dust and moisture.
Hacktintosh Systems
When this is present on a Hackintosh, it is often due to kernel extensions in the EFI System Partition (ESP) so go to the utility section for a mounting program so that you can see if USBInjectAll.kext is there. If missing, try and add it but on a bootable USB Flash drive so that your system drive remains untouched, and add to the system drive once you have confirmed no issues are present. If present, try skipping on boot or change the version to a known working version.
